The Centre d'études collégiales à Chibougamau offers adapted services for implementing an intervention and accommodation plan.

As part of my application for admission to the Centre d’études collégiales à Chibougamau (CECC), I have disclosed various personal information required to process this application. I understand that, in accordance with the Act Respecting Access to Documents Held by Public Bodies and the Protection of personal information, the CECC requires my authorization to transmit personal information about me that is necessary to confirm my enrollment. For this specific purpose, I authorize the CECC to electronically transfer to the ministère de l’Enseignement supérieur du Québec (Socrate System) the following personal information about me: last name(s), first name(s), admission number (DA number), the study program for which I have submitted an application and the courses I will be taking. This consent is valid for a period of six months. I understand that this consent may be modified or revoked at any time. To do so, I must inform a CECC representative.

To allow the adapted services team to contact me regarding my individualized intervention plan at the College, I authorize the Centre d'études collégiales à Chibougamau (CECC) to disclose the following personal information about me: last name(s), first name(s), and telephone number. This information will be communicated in writing to the Adapted Services Advisor solely for this purpose. This consent is valid for the duration of my studies at the CECC as well as for the retention period provided for under Québec’s Archives Act. I understand that I may modify or withdraw my consent at any time by informing the Adapted Services Advisor. However, withdrawing my consent may prevent the implementation of a potential intervention plan.

As part of my admission application to the Centre d'études collégiales à Chibougamau (CECC), I have disclosed various personal information required to process this application. For this specific purpose, I authorize the CECC to electronically transfer to the Service régional de l’admission des cégeps du Saguenay–Lac‑Saint‑Jean (SRASL) the following personal information about me: last name(s) and first name(s), admission number (DA number), and the study program for which I submitted an application. This consent is valid for a period of six months. I understand that this consent may be modified or revoked at any time. To do so, I must inform a CECC representative.
